Introduction to the Online Benefit Forms.  The links to the forms can be found under the Forms Section at the bottom of this page.

Please note that between 12:00 hours and 15:00 hours on 7 September 2010 the online claim forms will not be available.  Please accept our apologies for any inconvenience this may cause you.

Completing a benefit claim can be a problem for some people.

That is not only a problem for you but it is also a problem for us.

If we don't get the information we need at the outset, we have to write back or telephone you for the missing information. That means we cannot tell you the outcome as quickly as you would like.

We have used internet technology to make form filling very easy for you. When you use the forms for the first time it will take a minute or so for the PC to build the form. Please be patient.

  • You can fill in the whole form off-line.
  • You can save a part-filled form on your computer.
  • The form will help and guide you to ensure you only fill in parts that are relevant to your claim.
  • The form checks for errors, and highlights anything you have missed.
  • If you need them, supplementary forms for your employer and landlord can be printed.
  • When completed you can submit the form directly to our server, so we can begin processing your claim immediately.
  • The form will create a checklist to remind you of the additional evidence you must provide for us.

Completing the online form helps us to save money, time, and paper. Our online forms are secure. If you are in a public place (library, internet cafe, etc) and you are providing us with personal information, please take extra precautions to protect your information.

If you would prefer to fill in a paper copy of the form or would like help with the form, please contact the Money Matters team.
